Service Agreement

By signing this form, I acknowledge that I have read and understand the following:

• I am receiving nail services voluntarily
• I understand the nature of the services to be performed
• I have disclosed any allergies, skin conditions, or medical issues that may affect the service
• I understand that nail services may not be suitable for everyone
• I will inform my technician immediately if I experience any discomfort during the service

Health & Safety

For your safety and the safety of our staff, please note:

• We maintain strict sanitation and sterilization protocols
• All tools are properly cleaned and disinfected between clients
• If you have any contagious conditions, please reschedule your appointment
• We reserve the right to refuse service if we suspect a contagious condition

Liability Release

I understand and agree that:

• Elevation Nails and its staff are not responsible for any allergic reactions to products used
• I will not hold Elevation Nails liable for any pre-existing nail or skin conditions
• I understand that nail services are cosmetic and not medical treatments
• I will follow all aftercare instructions provided by my technician
